kukkiwon correction form

Hi all. have a student who came from another school and their name was spelled wrong on vert. Ive never dealt with this. is there a form to fill out and mail in? where would I find this form? any help would be appreciated.

According to the FAQ on the Kukkiwon web site, you simply ask that the certificate be re-issued and provide the corrected information.

I had to do this recently, and this is the e-mail I got from Kukkiwon's North American rep:

In order to fix it, you should send copy of passport and write down his certificate number on it. Remark "New certificate with birthday change. Mailing address include please.

it will be cost USD 15$( enclose in the envelope-payable personal check or money order)

you should send it to kukkiwon korea by airmail only.

I haven't gotten the new certificate yet, but airmail from US to Korea is like 2-3 weeks, and I only sent it a couple weeks ago. So we'll see.

I wrote a letter explaining the error and provided documentation: a photocopy of a driver license or passport.
