Yes it’s really awful. I had a friend who had to take six months off work due to it and she described the pain as…well…indescribable.
 
The independent immune system of the brain is really fascinating. I had reason to read up a bit on primed microglial cells, as that that seems to be one commong link to this issues, as well as the post-covid brain fog. Unlike the lymphatic clearance of the white blood cells, the brain seems very slow to recover, and i read that primed mikroglial cells, for exampe from post-covid can sustain for decades, and the effect is that the brains immuneresponse grossly overreacts to minor cytokine singnalling at the blood brain barrier. So even a systemtically mild infection, can make your brain behave like a serious inflammation. I think i have some of this still clininging... it was the reason why i looked into it. And in extrem cases it seems the brains immune response might border to slightly excitoxic even. But brainfog during cognitive load - as oppose to systemic excercise - makes it worse as cognotive processes compete with glutamate clearance. Deepest respect for the intricate evolved function of the brain and body. There is ALOT of research on mikroglial cells from different perspectives.
 
The brain actually has a Glymphatic system, discover only about 10 years ago. It’s used to flush gunk out of the brain.

Talking with my MD about it. Yes it sucks, and in some cases it can do permanent nerve damage. He told me that when he started in medicine he had no intention of getting the shingles vaccine. But after being in medicine for many years and dealing with some pretty severe cases, especially those that cause permanent damage, when he reached the proper age, he was getting the shingles vaccine

Which is why i got the vaccine, several years later than i should have, hut I have it now

I had a mild case for some I have heard of. Made a near perfect crescent from behind my right ear, over to my eyebrow. Hurt like a b***h when it started drying up.

But as far as Iunderstand it's not even close to as efficient as the lymphatic system? so mikroglial cells seems to stay primed for a long time, unlike the rest of the body. And this difference seems a probably key to understanding long-covid ethiology, although I think that is also ongoing resercher.

I don’t quite know how ‘efficiency’ would be measured in these cases, so I’m not sure. Each vessel of the glymphatic system is formed by foot projections of another glial cell-astrocytes creating a tube right around each cerebral blood vessel which would appear to be an optimal arrangement, whereas the lymphatic vessels are simple ‘close’ to blood vessels or generally imbedded in the tissues they drain.

It’s not an area I’m familiar with, to be honest, so my opinion is of no worth!
 
As far as i recall reading primed(hyper-reative) microglial cells arent cleared efficiently, instead their priming can be reinforced so they hang aroung for many years. They do relaxe, but much slower, and can be reactivated my mild infections... (so you get like reactivation of post-covid symptomts) years agot due to a simple cold... at least in some people... so this explain the weird cognitive symptoms.. although systemetic effects are almost neglectable... because the systemic immue response is "normal" but the brains immune response is "hyperactivated"... in some cases after post-covid... and can reamain hyperactive for many many years...
