It's not so bad, this guy creamed a $38 Billion dollar one:

JUNEAU, Alaska - Perhaps you know that sinking feeling when a single keystroke accidentally destroys hours of work. Now imagine wiping out a disk drive containing an account worth $38 billion. That’s what happened to a computer technician reformatting a disk drive at the Alaska Department of Revenue. While doing routine maintenance work, the technician accidentally deleted applicant information for an oil-funded account — one of Alaska residents’ biggest perks — and mistakenly reformatted the backup drive, as well.

Well, that's a state worker for you - I should know, I work for a state agency.
Too bad the back ups were also destroyed and they had to go to the paper files and reenter data.

I remember a couple of years ago, a unpaid student volunteer was working on another floor and was downloading loads of MP3 files onto the computer they were assigned. The volunteers supervisor found out and told them to remove the MP3 files. The volunteer unknowingly reformated several of our servers. We had to go to the back up tapes. At least it wasn't my work that was lost.
